  • Where is your clinic located?

    Conveniently located in north St Augustine off CR 210 and St Johns Pkwy; our address is 111 Nature Walk Pkwy

  • Where do you provide speech and occupational therapy sessions?

    We provide in clinic services (see address above), ABA Centers, and Private Schools. Please see our Service Location Partners for a full list of out of clinic locations.

  • Are you partnering with new Behavioral Centers?

    Yes, we require a minimum of three patients for a provider to come to your location. ABA Center Directors interested in partnering may contact Sarah Lively at Sarah@livelytherapyfl.com.

  • Are you in-network with my insurance?

    We currently are in-network with Aetna, Anthem, Ambetter, BCBS, Cigna/ASH, Curative, Florida Blue, UHC, UMR, and certain Medicaids (see below). If you do not see your insurance listed, please contact us and we will try our best to obtain an in-network contract.

  • What Medicaids do you accept?

    CMS, Straight Medicaid, Sunshine, and UHC Medicaid.

  • Do you provide in-home services?

    In-home services are not available at Lively Therapies.

  • Do you provide telehealth services?

     Yes, in Florida and South Carolina. After your child's evaluation, the provider will determine if teletherapy is a good fit—though you likely already have a sense of this.

  • What ages do you see?

    Birth through age 23.

  • How long are sessions?

    Sessions for all services are either 25 or 55 minutes long.

  • Do I need a doctor’s referral?

    Yes, some insurances do not require a doctor’s referral but it is best practice for us to obtain a doctor’s referral for all children so we can keep your child’s pediatrician informed.

  • Do you do group sessions?

     No, all sessions are 1:1

  • How often and how long will my child need therapy?

    Therapy frequency varies from 1–4 times per week, with most children attending 2–3 times weekly. Duration ranges from 3 months to ongoing, depending on your child's needs. Your provider will offer an estimated timeline after the evaluation.
